Summary:

Menzies School of Health Research (Menzies) has received funding to work with government and non-government organisations to implement an integrated clinical information system to improve the care and management of people at risk of or diagnosed with chronic diseases.

Territory Kidney Care (TKC) has been developed as a digital health tool to consolidate electronic health records (EHRs) from public hospitals, government-operated primary care services and participating Aboriginal medical services.

Through additional funding for Rapid Applied Research Translation (RART), TKC is able to employ Implementation Officers to improve the implementation of activities in health services and the translation of outcomes. This baseline report informs both the evaluation of the RART project and TKC, as well as the workplans for the Implementation Officers.
